NEWS:  We have now gone live with our new online consultation service - Patient Triage
This is a great way for you to contact our practice for non-urgent medical or admin requests.  It is very simple to fill-out so please give it a go if you need to contact us.  Although the character count is limited on the form, please try to give as much specific and concise information as possible. We really think it is easier for most people than waiting on hold to speak to a receptionist - but would love to hear your feedback. If you do not have access to the internet or find it difficult to use a computer or smartphone you can call reception instead.

You can now contact us about a MEDICAL, ADMINITRATIVE or PRESCRIPTION issue by clicking the graphic above or the button below. Patient triage is now also available on the NHS APP.  The short online form is the quickest and simplest way for the majority of our patients to access our clinical team for assistance. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.

PLEASE DO NOT USE THE ONLINE FORM FOR EMERGENCY REQUESTS. THE SUBMITTED FORMS WILL ONLY BE READ DURING OFFICE HOURS. The forms will not be read at weekends (Saturdays and Sundays) or on Bank Holidays. They will not be read Monday to Friday between the hours of 18.00 - 08.00. Please contact 111 during out of hours or 999 in case of medical emergencies.

SYSTM ONLINE

Book appointments and order repeat prescriptions

Patients can register for Systm Online. Once registered you can book appointments on-line, order prescriptions, view your detailed coded clinical record via the internet and request access to your full clinical record.

To do this, please ask at reception and bring a form of ID with you so that we can verify your identity. You can download the registration form from this website, but must bring this to the surgery with a form of ID as listed on the bottom of the registration form.

You will be given a username and password for the online service. Once registered, please click on the  logo above to access your Systm Online services.

   NHS App


With the NHS App, you can check your symptoms and get immediate advice. You can also book appointments at your GP practice, order repeat prescriptions, view your medical record and more.  To use the NHS App you must be aged 13 or over and registered with a GP surgery in England.

Keeping your data secure

After you download the app, you will need to set up an NHS login and prove who you are. The app then securely connects to information from your GP surgery.If your device supports fingerprint detection or facial recognition, you can use it to log in to the NHS App each time, instead of using a password and security code.
